Annual Questionnaire 2008

This document puts you, the member, at the centre of the Group’s planning for the provision of Aids, Appliances and Services for the year.  Not alone do you get a formal opportunity to assess your needs for the year but the information you supply forms a central part of the Group’s dealings with the Health Service Executive, the Department of Health and Children and the Government.  The results from the questionnaire allows the Group to present solid evidence of the real needs of Polio Survivors and this leads directly to the provision of the supports that you or other Polio Survivors need now, or that may be needed in the future.

Important Notice:

This information is being gathered on behalf of Polio Survivors resident in the Republic of Ireland only.  Any data supplied by others cannot be processed.
